Introduction

This Diabetes Risk Calculator estimates your chance of getting Type 2 diabetes. You answer 12 short questions. The tool adds up points and gives you a score out of 41. Then it tells you if your risk is low, moderate, high, or very high.

The questions cover two kinds of risk factors. Some you cannot change, like your age, sex, family history, and background. Others you can change, like your blood pressure, smoking, diet, exercise, and waist size. The calculator shows both groups side by side, so you can see what is driving your score.

You also get a "what if" number. It shows how much lower your score could be if you fixed the lifestyle items that added points. Along with that, you get a list of clear steps to take and a full breakdown of every point.

What Is Type 2 Diabetes Risk?

Type 2 diabetes happens when your body stops using insulin well. Insulin is the hormone that moves sugar from your blood into your cells. When it stops working right, sugar builds up in your blood. Over years, high blood sugar can hurt your eyes, kidneys, nerves, heart, and feet.

Type 2 diabetes builds up slowly. Many people have it for years before they know. That is why checking your risk early matters. More than 1 in 3 adults in the U.S. have prediabetes, and most of them do not know it.

Risk Factors You Cannot Change

Some things raise your risk no matter what you do. These are your non-modifiable risk factors:

  • Age. Risk climbs after 45 because cells respond to insulin less well as you get older.
  • Family history. If a parent, brother, sister, or child has type 2 diabetes, your risk goes up a lot.
  • Race and ethnicity. Hispanic, Black, Asian American, Native American, and Pacific Islander adults get type 2 diabetes more often.
  • Sex. Men tend to build harmful belly fat at a lower weight than women.
  • Gestational diabetes. High blood sugar during pregnancy raises the chance of type 2 diabetes later.

You cannot change these. But knowing them tells you how hard you need to work on the parts you can change.

Risk Factors You Can Change

These modifiable factors are the ones you can act on:

  • Prediabetes. Blood sugar that is high, but not high enough to be diabetes. Without action, many people with prediabetes get type 2 diabetes within 5 years. With action, many go back to normal.
  • Waist size. Fat around your belly is the kind that blocks insulin most. Risk starts rising past 35 inches (88 cm) for women and 40 inches (102 cm) for men.
  • Activity. Moving your muscles pulls sugar out of your blood without needing much insulin. 150 minutes a week of brisk walking is the common goal.
  • Food. Fruit and vegetables add fiber, which slows how fast sugar hits your blood.
  • Blood pressure. High blood pressure and insulin problems often show up together.
  • Smoking. Smokers are 30 to 40% more likely to get type 2 diabetes than non-smokers.

Why Your Score Matters

A risk score is a screening tool, not a diagnosis. It shows how many warning signs you have stacked up. A high score is a signal to get a simple blood test from a doctor or clinic, either an A1C test or a fasting glucose test. Only a blood test can tell you if you have prediabetes or diabetes.

Good News: Risk Can Drop

Type 2 diabetes is often preventable. In the Diabetes Prevention Program, a large U.S. trial, people with prediabetes who lost just 5 to 7% of their body weight and walked 30 minutes a day, 5 days a week, cut their chance of getting type 2 diabetes by about 58%. For adults over 60, the drop was even bigger. Small steps, done often, work better than big changes you cannot keep.

Frequently asked questions

What blood sugar numbers mean prediabetes or diabetes?

Doctors use two common blood tests. Here are the cut-offs:

  • A1C test: normal is below 5.7%. Prediabetes is 5.7% to 6.4%. Diabetes is 6.5% or higher.
  • Fasting blood sugar: normal is below 100 mg/dL. Prediabetes is 100 to 125 mg/dL. Diabetes is 126 mg/dL or higher.

A risk score cannot give you these numbers. Only a blood test can.

Can prediabetes be reversed?

Yes, in many cases. Prediabetes is not permanent. Losing 5% to 7% of your body weight, walking 30 minutes a day, and eating more fiber can bring blood sugar back to normal for many people.

The sooner you act, the better it works. Without changes, many people with prediabetes go on to get type 2 diabetes.

How do you measure your waist correctly?

Follow these steps:

  1. Stand up straight and take off bulky clothes.
  2. Put a tape measure around your bare belly, level with your belly button.
  3. Keep the tape snug but not tight. Do not squeeze the skin.
  4. Breathe out normally, then read the number.

Do not measure over your pants or suck in your stomach. That gives a false low number.

What waist size raises your risk of type 2 diabetes?

Risk starts rising at these points:

  • Women: over 31.5 inches (80 cm), and much higher over 35 inches (88 cm).
  • Men: over 37 inches (94 cm), and much higher over 40 inches (102 cm).

Belly fat wraps around your organs and blocks insulin from working. That is why waist size matters more than weight alone.

At what age should you get tested for type 2 diabetes?

Most guidelines start screening at age 35 for all adults. Start earlier if you are overweight and have another risk factor, like family history, high blood pressure, or past gestational diabetes.

Children and teens with obesity plus other risk factors may also need testing.

How often should you check for diabetes?

If your test result is normal, repeat it every 3 years. If you have prediabetes, get tested every year. If you had gestational diabetes, get tested every 1 to 3 years for life.

Your doctor may test you more often if your risk is high.

Does eating sugar cause type 2 diabetes?

Not directly. Eating sugar does not give you diabetes by itself. But sugary drinks and snacks add lots of calories, which leads to weight gain and belly fat. That is what drives insulin resistance.

Sugary drinks are the worst offender because they are easy to drink fast and do not fill you up.

Can you get type 2 diabetes if you are not overweight?

Yes. About 1 in 10 people with type 2 diabetes are at a normal weight. Fat stored around the liver and organs can cause insulin problems even when the scale looks fine.

Family history, age, and ethnic background also matter. Asian adults in particular can develop type 2 diabetes at a lower body weight than other groups.

What is the difference between type 1 and type 2 diabetes?

Type 1 happens when the immune system destroys the cells that make insulin. The body makes almost none. It often starts in childhood and needs insulin shots for life. It cannot be prevented.

Type 2 happens when the body stops responding to insulin well. It builds up slowly, usually in adults, and is strongly linked to weight, activity, and genes. It can often be delayed or prevented.

About 90% to 95% of all diabetes is type 2.

If my parent has type 2 diabetes, will I get it?

No, it is not certain. Having a parent, brother, sister, or child with type 2 diabetes roughly doubles or triples your risk, but genes are only part of the story.

Families also share meals, habits, and activity levels. Changing those habits can push your risk down a lot, even with strong family history.

How long does prediabetes take to turn into type 2 diabetes?

Without changes, about 5% to 10% of people with prediabetes develop type 2 diabetes each year. Many progress within 5 to 10 years.

It is not a straight line, though. Diet, exercise, and weight loss can stop it or even push blood sugar back to normal.

Does poor sleep raise your diabetes risk?

Yes. Regularly sleeping less than 6 hours a night raises blood sugar and makes your body less sensitive to insulin. Poor sleep also makes you hungrier and more likely to eat high-carb foods.

Sleep apnea, which causes snoring and pauses in breathing, is strongly linked to type 2 diabetes. Aim for 7 to 9 hours a night.

What kind of exercise lowers blood sugar best?

Both types help, and together they work best:

  • Brisk walking or cycling burns blood sugar during and after the activity. Aim for 150 minutes a week.
  • Strength training builds muscle, and muscle stores more sugar. Do it 2 days a week.

A 10 to 15 minute walk right after meals is one of the easiest ways to blunt a blood sugar spike.

Does gestational diabetes go away after the baby is born?

Blood sugar usually returns to normal within weeks of birth. But the risk does not disappear. About half of women who had gestational diabetes develop type 2 diabetes within 5 to 10 years.

Get a blood sugar test 4 to 12 weeks after delivery, then every 1 to 3 years after that.

Can type 2 diabetes go into remission?

Yes, for some people. Remission means blood sugar stays in the normal range without diabetes medicine. It is most likely in people who lost a large amount of weight and who were diagnosed recently.

Remission is not a cure. Blood sugar can rise again, so you still need regular check-ups.
