Introduction

Most of the testosterone in your blood is stuck to proteins. About two-thirds sticks tightly to a protein called SHBG (sex hormone-binding globulin). Most of the rest sticks loosely to albumin. Only a small part floats free. That free part is what your body can actually use.

A normal blood test usually shows total testosterone, which adds all of it together. That number can look fine even when your free testosterone is low. This happens a lot when SHBG is high or low, such as with aging, thyroid problems, obesity, or liver disease.

This free testosterone calculator works out your free and bioavailable testosterone from three lab values: total testosterone, SHBG, and albumin. It uses the published formulas from Vermeulen, Ly, and Södergård, and shows all three side by side so you can compare them.

How to use our Free Testosterone Calculator

What Is Free Testosterone?

Testosterone travels in your blood in three forms. Most of it is stuck to a protein called SHBG (sex hormone-binding globulin). That part is locked up and your body cannot use it. Some is loosely held by albumin, another blood protein. And a small part, about 1% to 3% in men, floats on its own. That loose part is called free testosterone.

Free testosterone is the form that can enter your cells and do work. It helps build muscle and bone, supports mood and energy, and drives sex drive and sperm production.

Free vs. Bioavailable vs. Total Testosterone

  • Total testosterone: everything in your blood, bound and unbound.
  • Free testosterone: the unbound part your body can use right away.
  • Bioavailable testosterone: free testosterone plus the albumin-bound part. Albumin holds on weakly, so this hormone can still be used.

Why SHBG and Albumin Matter

Your total testosterone can look normal while your free testosterone is low. This happens when SHBG is high, because more hormone gets locked away. SHBG often rises with age, liver disease, thyroid problems, and some medicines. SHBG drops with obesity, type 2 diabetes, and insulin resistance, which can make total testosterone look low even when free testosterone is fine. That is why doctors check total testosterone, SHBG, and albumin together.

How Free Testosterone Is Estimated

Measuring free testosterone directly is hard and costly. Instead, labs and doctors use math formulas based on how hormones bind to proteins. The best known is the Vermeulen formula from 1999. The Södergård formula uses slightly different binding numbers, and the Ly formula treats SHBG as having two binding spots instead of one. Each gives a slightly different answer, so it helps to compare them.

Typical Levels

In healthy men, free testosterone is usually around 50 to 200 pg/mL, and it slowly falls after about age 30. In women, levels are far lower, normally under about 7 pg/mL. Normal ranges change with age, sex, and the lab doing the test.

Signs of Low Free Testosterone

Low levels can cause tiredness, low sex drive, trouble building muscle, weight gain, poor sleep, low mood, and trouble focusing. In men, it can also cause erection problems and fertility trouble. In women, high testosterone is more common and can cause acne, extra body hair, and missed periods.

Getting an Accurate Test

Testosterone is highest in the morning. Have blood drawn between 7 a.m. and 10 a.m., and fast if your doctor asks. Because levels move day to day, a low result should be checked with a second test on another morning before any diagnosis is made.

What is a normal free testosterone level for a man?

In most healthy men, free testosterone falls between about 50 and 200 pg/mL (roughly 175 to 700 pmol/L). The range shifts down with age. A man in his 20s may sit near 150 pg/mL, while a man in his 70s may be closer to 60 pg/mL and still be normal for his age.

Every lab sets its own range, so compare your number to the range printed on your own report.

What is a low free testosterone level?

Many doctors treat free testosterone under about 65 pg/mL (225 pmol/L) in adult men as low. Some labs use 50 pg/mL as the cutoff.

A single low number is not a diagnosis. Testosterone changes day to day, so a low result should be repeated on another morning before any treatment starts.

How do you calculate free testosterone from total testosterone and SHBG?

You cannot use simple division. Free testosterone is found with a mass-action equation that models how testosterone binds to SHBG and albumin. The steps are:

  1. Change total testosterone and SHBG to nmol/L and albumin to mol/L.
  2. Work out the albumin factor: N = 1 + (Kalb × albumin).
  3. Solve a quadratic equation using the SHBG binding constant.

The answer comes out in mol/L, then gets multiplied by 288.42 to give pg/mL.

What is the Vermeulen formula for free testosterone?

It is a 1999 method by Vermeulen, Verdonck, and Kaufman that estimates free testosterone from total testosterone, SHBG, and albumin. It assumes SHBG has one testosterone binding site, with a binding constant of 1.0 × 10⁹ L/mol for SHBG and 3.6 × 10⁴ L/mol for albumin.

It is the most used method in clinics and research, and it matches measured values from equilibrium dialysis fairly well.

How much of your testosterone is free?

Only about 1% to 3% in men and about 0.5% to 2% in women. The rest is bound:

  • About 44% to 65% is stuck tightly to SHBG.
  • About 33% to 54% is held loosely by albumin.

So out of a total testosterone of 500 ng/dL, only around 10 ng/dL is truly free.

What is bioavailable testosterone?

Bioavailable testosterone is your free testosterone plus the part stuck to albumin. Albumin holds on weakly, so that hormone can break loose and enter tissue.

It usually makes up 35% to 55% of total testosterone. Some doctors prefer it over free testosterone because it shows everything the body can actually reach.

Why is my total testosterone normal but my free testosterone low?

Almost always because of high SHBG. SHBG locks up testosterone, so a big share of your total is unusable. Your total number looks fine, but very little is free.

Things that raise SHBG include aging, liver disease, an overactive thyroid, estrogen, anticonvulsants, and being very lean or underfed.

What causes low SHBG?

Common causes are obesity, type 2 diabetes, insulin resistance, an underactive thyroid, fatty liver disease, and steroid or growth hormone use.

When SHBG is low, your total testosterone can read low while free testosterone stays normal. That is why testing SHBG alongside total testosterone matters.

How do you convert free testosterone from pg/mL to pmol/L?

Multiply pg/mL by 3.467 to get pmol/L. To go the other way, divide pmol/L by 3.467.

Example: 100 pg/mL × 3.467 = 347 pmol/L.

The number comes from testosterone's molecular weight of 288.42 g/mol.

How do you convert total testosterone from ng/dL to nmol/L?

Multiply ng/dL by 0.0347. To go back, multiply nmol/L by 28.84.

Example: 500 ng/dL × 0.0347 = 17.3 nmol/L.

US labs mostly report ng/dL. Labs in Europe, Canada, and Australia mostly report nmol/L.

Do I need an albumin test to calculate free testosterone?

It helps, but it is not critical. Albumin changes free testosterone only a little compared to SHBG. Most calculations use a default of 4.3 g/dL when the real value is unknown.

Use your real albumin if you have liver disease, kidney disease, or malnutrition, since those conditions can drop it well below normal and change the result.

When is the best time to get a testosterone blood test?

Between 7 a.m. and 10 a.m. Testosterone peaks in the early morning and can fall 20% to 30% by afternoon, especially in younger men.

Skip hard workouts the day before, and do not test while sick or in hospital. If your doctor asks, fast beforehand, since eating can lower testosterone for a few hours.

Why do different formulas give different free testosterone results?

Each formula uses different binding constants and assumptions. The Ly method treats SHBG as a dimer with two binding sites, so it usually returns a lower free testosterone than Vermeulen. Södergård uses a weaker SHBG constant (5.97 × 10⁸), which often gives a slightly higher result.

Differences of 10% to 30% between methods are normal. Stick with one method when tracking changes over time.

Is calculated free testosterone as good as a direct lab test?

Calculated free testosterone matches equilibrium dialysis, the gold standard, quite closely and costs far less. Major guidelines accept it.

The cheap analog free testosterone immunoassay that some labs run is not reliable and should be avoided. Ask for total testosterone plus SHBG and albumin instead.

What are the symptoms of low free testosterone?

Common signs in men include low sex drive, weak erections, tiredness, loss of muscle, more belly fat, low mood, poor focus, and fewer morning erections.

In women, low levels can cause low libido, fatigue, and low mood. These symptoms overlap with thyroid problems, depression, anemia, and sleep apnea, so lab testing is needed to confirm.

Does free testosterone drop with age?

Yes. Free testosterone falls about 1% to 2% per year after age 30. That is faster than total testosterone, because SHBG rises with age and locks more hormone away.

A man at 70 may have a total testosterone similar to a man at 40, but much less free testosterone.

What is a normal free testosterone level for a woman?

Usually under about 7 pg/mL (roughly 0.3 to 6.8 pg/mL), with lower values after menopause. Total testosterone in women runs about 15 to 70 ng/dL.

High free testosterone in women is more common than low and often points to PCOS. It can cause acne, extra facial or body hair, hair thinning, and irregular periods.

How can I raise my free testosterone naturally?

These changes have the strongest evidence behind them:

  • Lose extra body fat, which lowers how much testosterone turns into estrogen.
  • Sleep 7 to 8 hours; most testosterone is made during sleep.
  • Lift weights and do resistance training.
  • Fix low vitamin D, zinc, and magnesium if you are deficient.
  • Cut back heavy drinking and long-term stress.

Weight loss also lowers SHBG less than most people expect, so results vary. See a doctor if levels stay low.

Can medications affect free testosterone results?

Yes. Opioids, steroids like prednisone, and some antidepressants lower testosterone. Estrogen, birth control pills, thyroid hormone, and anticonvulsants raise SHBG, which cuts free testosterone.

Biotin supplements can also mess up some hormone immunoassays. Stop biotin 2 to 3 days before testing and tell your doctor every drug and supplement you take.
