Introduction
The Mentzer Index is a quick screening tool that helps tell apart two common causes of small red blood cells: iron deficiency anemia (IDA) and beta thalassemia trait (BTT). Both show up on a blood test as low MCV, so they can look alike at first.
This calculator uses two numbers from a standard complete blood count (CBC): your MCV (the average size of a red blood cell) and your RBC count (how many red blood cells you have). It divides MCV by RBC to give one simple number.
Here is what that number means:
- Less than 13: points toward beta thalassemia trait
- Exactly 13: borderline, more testing is needed
- More than 13: points toward iron deficiency anemia

How to use our Mentzer Index Calculator
Enter two values from your complete blood count (CBC) report: your MCV and your RBC count. The calculator divides MCV by RBC and shows your Mentzer Index, a step-by-step solution, and what the score may point to, either beta thalassemia trait or iron deficiency anemia.
MCV (Mean Corpuscular Volume): Type the MCV number from your blood test, such as 70. Then pick the unit next to the box, fL or µm³. Both units mean the same thing, so the number stays the same. A normal MCV is about 80 to 100 fL.
RBC Count (Red Blood Cell Count): Type your red blood cell count, such as 5.2. Then choose the unit button that matches your lab report: ×106/µL, ×1012/L, or cells/µL. If you switch units, the calculator changes your number for you. A normal RBC count is about 4.0 to 6.0 ×106/µL.

What Is the Mentzer Index?

You divide the MCV (mean corpuscular volume, the average size of one red blood cell) by the RBC count (how many red blood cells you have).
The Formula
Mentzer Index = MCV (fL) ÷ RBC count (×106/µL)
The answer has no units. It is just a ratio.
How to Read the Result
- Less than 13: points toward beta thalassemia trait.
- Exactly 13: borderline. The test cannot tell which one it is.
- More than 13: points toward iron deficiency anemia.
Why It Works
Both conditions make red blood cells small (low MCV), but the body reacts in different ways. In beta thalassemia trait, the bone marrow still makes many red blood cells, they are just small. A high RBC count with a low MCV gives a small index. In iron deficiency, the body runs out of iron, so it makes fewer red blood cells and they are small too. A low RBC count with a low MCV gives a bigger index.
How Good Is It?
At a cut-off of 13, the Mentzer Index catches most cases. It is about 91% to 98% sensitive for iron deficiency anemia, and about 83% to 93% specific. It is one of the most accurate of the many red cell index formulas, and it is the easiest to work out.
Important Limits
This is a screening tool, not a diagnosis. It only helps when red blood cells are already small (MCV under 80 fL). It can give the wrong answer if a person has both problems at once, is pregnant, has alpha thalassemia, or has another long-term illness. To be sure, doctors order iron studies (ferritin, serum iron, TIBC) for iron deficiency and hemoglobin electrophoresis with HbA₂ levels for thalassemia. Always talk to a doctor about your results.
