What Is the Spherical Equivalent?

The spherical equivalent (SE) turns a full glasses prescription into one single number. Many prescriptions have two parts: sphere (SPH) and cylinder (CYL). The sphere fixes blur that is the same in every direction. The cylinder fixes astigmatism, which is blur caused by an eye shaped more like a football than a basketball. The SE mixes both into one power, measured in diopters (D).

The Formula

SE = SPH + (CYL ÷ 2)

You add half of the cylinder to the sphere. For example, a prescription of −2.00 −1.00 × 180 has an SE of −2.50 D. The math works because half the cylinder power sits at the middle point between the eye's two focus lines. That middle point is called the circle of least confusion.

Why the Axis Does Not Matter Here

The axis (1° to 180°) tells you where the cylinder is placed, not how strong it is. Since SE only cares about power, the axis is not used in the math. It still belongs on the prescription, so keep it written down.

Where People Use It

  • Contact lenses: Some soft lenses come in sphere power only. The SE gives a close starting power when the astigmatism is small (about 0.75 D or less).
  • Vision screenings: One number is faster to compare than two.

Reading the Result

A negative SE means myopia (nearsighted): far things look blurry. A positive SE means hyperopia (farsighted). An SE near zero means little or no refractive error. Common groupings are:

  • −0.50 to +0.50 D: emmetropia, no big error
  • −0.50 to −3.00 D: mild myopia
  • −3.00 to −6.00 D: moderate myopia
  • Worse than −6.00 D: high myopia
  • +0.50 to +2.00 D: mild hyperopia

Difference Between the Eyes

When the SE of the right eye (OD) and left eye (OS) differ by 1.00 D or more, it is called anisometropia. The brain can struggle to blend two very different images. In kids this can lead to a lazy eye (amblyopia). A gap like this is worth showing to an eye doctor.

Minus and Plus Cylinder Notation

The same prescription can be written two ways. Optometrists often use minus cylinder; ophthalmologists often use plus cylinder. To switch, add the cylinder to the sphere, flip the cylinder sign, and turn the axis 90°. For example, −2.00 −1.00 × 180 equals −3.00 +1.00 × 090. Both mean the same lens, and the spherical equivalent stays the same either way.

What It Cannot Do

The SE is a summary, not a replacement. It drops the astigmatism correction, so glasses made from SE alone will look softer for anyone with real astigmatism. It also ignores prism, add power for reading, and how the lens sits from your eye. Use the SE as a quick guide, and get a real eye exam for anything you plan to wear.

Frequently asked questions

What is the spherical equivalent of -3.00 -1.50 x 180?

It is -3.75 D.

Use the formula SE = SPH + (CYL ÷ 2):

  • Half of -1.50 is -0.75
  • -3.00 + (-0.75) = -3.75 D

The axis (180°) is not part of the math.

What is MRSE on a LASIK or cataract surgery report?

MRSE stands for manifest refraction spherical equivalent. It is the same SE number, but taken from the manifest refraction, the exam where you answer "better one or better two" without dilating drops.

Surgeons use MRSE to compare vision before and after surgery. A result close to 0.00 D means the eye is near the target.

What do OD, OS, and OU mean on a glasses prescription?

They are Latin short forms:

  • OD (oculus dexter) = right eye
  • OS (oculus sinister) = left eye
  • OU (oculus uterque) = both eyes

Some offices now just write RE and LE, or Right and Left.

What is a diopter?

A diopter (D) measures lens power. It equals 1 divided by the focal length in meters.

A 1.00 D lens focuses light at 1 meter. A 2.00 D lens focuses at 0.5 meters, so it is twice as strong. Eye prescriptions move in 0.25 D steps because smaller changes are hard to notice.

How much astigmatism needs a toric contact lens?

Most fitters switch to a toric lens at about 0.75 D or more of cylinder.

Below that, a plain sphere lens set to the spherical equivalent usually feels clear. Above that, the leftover blur starts to bother people, especially at night or when reading small print.

Is a contact lens power the same as the glasses spherical equivalent?

Not always. Contacts sit right on the eye, while glasses sit about 12 mm away. That gap changes the power needed.

Under about ±4.00 D, the numbers are basically the same. Above that, the power must be adjusted. For example, -6.00 D in glasses is about -5.60 D in contacts. Strong plus powers go up instead.

Does the spherical equivalent include the reading ADD?

No. The SE only uses the distance sphere and cylinder.

The ADD is extra plus power for reading, listed on its own line for bifocals or progressives. If you need the near power, add the ADD to the distance SE after you calculate it.

Can the spherical equivalent be 0.00 if you still have astigmatism?

Yes, and this trips people up. A prescription of +1.00 -2.00 x 090 has an SE of 0.00 D, but that eye is not clear without glasses.

The SE only shows the average power. It hides the astigmatism, so a person can have an SE of zero and still see blurry at every distance.

Can the spherical equivalent be positive when the sphere is negative?

Yes. If the cylinder is a large plus number, half of it can outweigh a minus sphere.

Example: -1.00 +3.00 x 180 gives SE = -1.00 + 1.50 = +0.50 D. On average, the eye is slightly farsighted.

How blurry is -2.00 D of myopia?

With -2.00 D, your clear point is about 0.5 meters (20 inches) away. Anything past that gets fuzzy, so faces across a room and road signs are hard to read.

A quick rule: divide 1 by your minus number to find the farthest clear distance in meters. -4.00 D means clear to only 0.25 m.

How many diopters make you legally blind?

Legal blindness is not measured in diopters. In the U.S. it means vision of 20/200 or worse in the better eye with the best glasses or contacts, or a visual field of 20 degrees or less.

Someone with -12.00 D who sees 20/20 in glasses is not legally blind, even though the number sounds high.

Does the spherical equivalent change as you get older?

Often, yes. Myopia usually grows during childhood and the teen years, then settles in the early twenties. Hidden farsightedness tends to show up more after age 40.

Presbyopia, the trouble reading up close after 40, does not change your distance SE. It only adds reading power.

What does DS or plano mean on a prescription?

DS means diopters sphere. There is no cylinder and no axis for that eye. You may also see SPH, or just a blank space.

Plano (or PL) means zero power. When the cylinder is 0.00, the sphere and the spherical equivalent are the same number.
