How to Use Our MELD 3.0 Calculator
Enter your patient's age group, sex, dialysis status, and five lab values below. The calculator will return a MELD 3.0 score, estimated 90-day survival rate, 90-day mortality rate, and a full step-by-step breakdown of the math.
Age Eligibility: Select whether the patient is 12 years of age or older. You can use our age calculator if you need to confirm the patient's exact age. This calculator is only valid for patients aged 12 and up. If the patient is under 12, use the PELD calculator instead.
Adult or Adolescent: Select whether the patient is 18 or older. Both age groups use the same additive constant of +6.00 in the MELD 3.0 formula.
Sex at Birth: Choose male or female. Female sex adds 1.33 points to the score. This reflects differences in waitlist outcomes between sexes.
Dialysis Status: Select "Yes" if the patient had dialysis at least twice in the past week or had continuous venovenous hemodialysis (CVVHD) for 24 or more hours. If yes, creatinine is automatically set to 3.0 mg/dL.
Serum Creatinine (mg/dL): Enter the patient's creatinine level. Normal range is 0.7–1.3 mg/dL. The calculator caps this value between 1.0 and 3.0 mg/dL. For a more detailed assessment of renal function, consider using the creatinine clearance calculator or the GFR calculator.
Serum Bilirubin (mg/dL): Enter the patient's total bilirubin level. Normal range is 0.3–1.9 mg/dL. Values below 1.0 are raised to 1.0 for the calculation. There is no upper cap.
INR: Enter the patient's International Normalized Ratio. Normal range is 0.8–1.2. Values below 1.0 are raised to 1.0 for the calculation. There is no upper cap.
Serum Sodium (mEq/L): Enter the patient's sodium level. Normal range is 136–145 mEq/L. The calculator caps this value between 125 and 137 mEq/L. If you need to evaluate the patient's overall osmolar status, the serum osmolality calculator can provide additional clinical context.
Serum Albumin (g/dL): Enter the patient's albumin level. Normal range is 3.5–5.5 g/dL. The calculator caps this value between 1.5 and 3.5 g/dL. Albumin is unique to MELD 3.0 and is not used in older MELD versions.

How Is MELD 3.0 Different from Older Versions?
MELD 3.0 was introduced in 2022 and replaced the older MELD-Na formula. It added two new factors: serum albumin and sex assigned at birth. Studies showed that women with liver disease had worse outcomes than men at the same old MELD score. By adding sex and albumin, MELD 3.0 predicts 90-day survival more fairly and more accurately for all patients. What Lab Values Does MELD 3.0 Use?
The MELD 3.0 formula uses five blood test results plus the patient's sex:
- Serum creatinine – shows how well the kidneys are working. A GFR calculator can offer a more complete picture of kidney function when evaluating patients with liver disease.
- Serum bilirubin – shows how well the liver clears waste from the blood. Elevated bilirubin alongside abnormal liver enzymes may also warrant evaluation with an AST/ALT ratio calculator.
- INR (International Normalized Ratio) – shows how well the blood clots
- Serum sodium – a low level can signal advanced liver disease
- Serum albumin – a protein made by the liver; low levels mean the liver is struggling
- Sex at birth – female patients receive an added 1.33 points
Each value has caps or floors. For example, creatinine is kept between 1.0 and 3.0 mg/dL, and sodium is kept between 125 and 137 mEq/L. If a patient has been on dialysis at least twice in the past week, creatinine is automatically set to 3.0 mg/dL.
Who Is This Calculator For?
This calculator is meant for patients aged 12 and older. Children under 12 should be scored using the PELD (Pediatric End-Stage Liver Disease) formula instead. Both adults (18+) and adolescents (12–17) use the same MELD 3.0 equation with an additive constant of 6.
What Do the Scores Mean?
A MELD 3.0 score gives an estimate of how likely a patient is to survive the next 90 days without a transplant. Scores below 10 are considered low risk, with over 98% survival. Scores of 40 mean the disease is critical, with roughly 71% or higher chance of death within three months. This score is one of the most important tools used by the United Network for Organ Sharing (UNOS) to allocate donor livers in the United States.
