Introduction

The PSA Density Calculator shows how much PSA you have for the size of your prostate. PSA is a protein made by the prostate. A blood test measures it. But a big prostate can make more PSA on its own, even with no cancer. That is why doctors also look at PSA density (PSAD).

The math is simple. You divide your total PSA by your prostate volume:

PSA density = total PSA (ng/mL) ÷ prostate volume (cc)

This tool works in two ways. If you have an MRI or ultrasound report with three measurements (length, width, and height), use Dimensional Mode. The calculator finds your prostate volume with the ellipsoid formula (L × W × H × 0.5236), the same math used by our Prostate Volume Calculator. You get your PSA density right away, plus a risk tier. A PSAD under 0.10 is low. Between 0.10 and 0.15 is borderline. Above 0.15 may point to a higher chance of prostate cancer, and your doctor may talk to you about a biopsy.

How to use our PSA Density Calculator

Enter your prostate size and your total PSA blood level. The calculator gives you your prostate volume, your PSA density in ng/mL/cc, and what that number means for prostate cancer risk.

Pick your mode: Choose Dimensional Mode if your MRI or TRUS report lists three prostate measurements. Choose Direct Volume Mode if your report already gives a prostate volume.

Length (AP): Type the front-to-back size of the prostate from the sagittal view. Pick cm, mm, or inches from the drop-down next to it.

Width (LR): Type the widest left-to-right size from the axial view, then pick the unit.

What Is PSA Density?

PSA density (PSAD) compares the amount of PSA in your blood to the size of your prostate. PSA stands for prostate-specific antigen. It is a protein made by the prostate gland. A blood test shows how much of it you have, in ng/mL.

The problem with PSA alone is that a big prostate makes more PSA, even when nothing is wrong. Many older men have an enlarged prostate (called BPH, or benign prostatic hyperplasia). Their PSA can look high just because the gland is large. PSA density helps sort this out by dividing the PSA level by the prostate volume.

The Formula

PSA density = total PSA (ng/mL) ÷ prostate volume (cc)

The answer is written in ng/mL/cc. One cc is the same as one mL. It is a ratio, much like the cholesterol ratio or the BUN/creatinine ratio used in other lab work — a single number that adds context to a raw result.

How Prostate Volume Is Measured

Doctors get prostate volume from an MRI scan or a transrectal ultrasound (TRUS). Sometimes the report gives the volume right away. Other times it lists three measurements: length (front to back), width (left to right), and height (top to bottom). Volume is then found with the ellipsoid formula, because the prostate is shaped a bit like an egg:

Volume = length × width × height × 0.52

What the Numbers Mean

  • Under 0.10 ng/mL/cc — Lower chance of prostate cancer. A high PSA is more likely from gland size.
  • 0.10 to 0.15 ng/mL/cc — Borderline. Doctors look at other facts, like age, family history, exam findings, and MRI results.
  • Above 0.15 ng/mL/cc — Higher chance of prostate cancer. Doctors often use this as a reason to talk about a biopsy.

Why Doctors Use It

PSA density can cut down on biopsies that are not needed. A man with a PSA of 6 ng/mL and a 60 cc prostate has a PSAD of 0.10. That is reassuring. A man with the same PSA of 6 ng/mL but a 25 cc prostate has a PSAD of 0.24. That result is more worrying, even though both blood tests read the same. PSAD is also used in active surveillance, where low-risk cancer is watched over time instead of treated right away. Things to Keep in Mind

PSA density is one clue, not an answer. Other things can push PSA up for a short time, such as a urine infection, prostatitis, a recent catheter, riding a bike, or ejaculation. Some medicines, like finasteride and dutasteride, lower PSA by about half. Volume numbers can also differ between MRI and ultrasound, and between two people reading the same scan. Always go over your results with a urologist, who will look at your PSA trend, exam, MRI score, and health history together. Frequently asked questions

Is cc the same as mL for prostate volume?

Yes. One cc equals one mL. A 40 cc prostate and a 40 mL prostate are the same size. So ng/mL/cc and ng/mL/mL mean the same thing.

My report lists prostate weight in grams. Can I use that number?

Yes. Prostate tissue weighs about the same as water, so 1 gram is about 1 cc. Type the gram number into Direct Volume Mode and leave the unit set to cc (mL).

Why is the volume here different from the volume on my MRI report?

This tool uses the ellipsoid formula (L × W × H × 0.5236). Many radiologists trace the gland slice by slice instead, which is called planimetry. That method often gives a slightly smaller volume. If your report already states a volume, use Direct Volume Mode so the numbers match.

Should I enter total PSA or free PSA?

Enter total PSA. PSA density is built on the total value from your blood test. Free PSA and percent-free PSA are separate tests with their own cutoffs and do not belong here.

Does a PSA density above 0.15 mean I have cancer?

No. It only means your risk is higher than average. Many men above 0.15 have no cancer. Only a biopsy can confirm prostate cancer.

Can PSA density look normal even if cancer is there?

Yes. A low PSAD lowers the odds but does not rule cancer out. Some cancers make little PSA. Your doctor still looks at your exam, MRI, and PSA trend over time.

Why does the chart line drop as prostate volume goes up?

Because density is PSA divided by volume. If PSA stays the same and the gland is larger, the density falls. The curve shows what your PSAD would be at other volumes, with your own result marked on it.

Is there a different PSA density cutoff for each age?

No. The 0.15 ng/mL/cc line is used for adult men of all ages. Older men often have bigger glands, so a high PSA can still give a low density.

Do I need to fast before a PSA blood test?

No, fasting is not needed. But for about 48 hours before the test, avoid ejaculation, long bike rides, and rectal exams. These can push PSA up and make your density look worse than it is.

How long should I wait after a biopsy or infection to test PSA?

Wait about 6 weeks after a prostate biopsy or a catheter, and wait until a urine infection or prostatitis is fully treated. PSA can stay high for weeks and would skew your density.

My PSA density is borderline, between 0.10 and 0.15. What happens next?

Your urologist puts it with other facts: how fast your PSA is rising, the rectal exam, family history, and the MRI PI-RADS score. Some men get an MRI first, some get a repeat PSA in a few months, some go to biopsy.

What is transition zone PSA density?

Some reports divide PSA by the volume of the inner transition zone only, not the whole gland. That is called PSAD-TZ and it uses different cutoffs. This calculator uses whole-gland volume.

Can I use this calculator after prostate surgery or radiation?

No. TURP, radiation, and prostate removal change or remove gland tissue, so the usual density cutoffs no longer fit. After treatment, doctors follow the raw PSA level and how it changes.

Do the PSA test and the scan need to be from the same time?

They should be close together, ideally within a few months. PSA moves month to month and the gland grows slowly, so old numbers mixed with new ones give a misleading density.

Which units can I type in?

  • Size: cm, mm, or inches
  • Volume: cc (mL), L, in³, or mm³
  • PSA: ng/mL, µg/L, ng/dL, or ng/L

Why does the tool also show ng/mL/in³?

It is only a unit swap for people who work in inches. Doctors report PSA density in ng/mL/cc, and every risk tier in this tool is based on the cc value.

Which measurement is length and which is height?

Length (AP) is front to back on the side (sagittal) view. Width (LR) is the widest side-to-side size on the axial view. Height (SI) is base to apex, top to bottom. Swapping them will not change the volume, but matching your report keeps things clear.

Does this calculator save or send my health data?

No. All the math runs in your browser. Nothing you type is stored or sent anywhere.

Why does my friend have the same PSA as me but a different risk tier?

Because your prostates are different sizes. A PSA of 6 ng/mL in a 60 cc gland gives a PSAD of 0.10, which is low. The same PSA in a 25 cc gland gives 0.24, which is elevated. Gland size is what changes the answer.
