Introduction

The Prostate Volume Calculator tells you how big a prostate gland is. You enter three measurements from an ultrasound or MRI scan (length, width, and height), and the tool calculates the volume.

It gives you two answers. The ellipsoid volume uses the classic formula (π/6 × L × W × H). The bullet volume uses a slightly different formula (5π/24 × L × W × H) that models the gland as a bullet shape, rounded at one end. The ellipsoid value drives the size classification and PSA density here, because those reference ranges were built on standard ellipsoid measurements; the bullet value is shown alongside for comparison.

The calculator also sorts the result into a size group: normal (under 30 mL), mildly enlarged (30–50 mL), moderately enlarged (50–80 mL), or significantly enlarged (80 mL and up). An enlarged prostate is linked to BPH (benign prostatic hyperplasia), which can make it hard to pee.

If you add a PSA blood test number, the tool also works out PSA density. This is PSA divided by prostate volume. The European Association of Urology's guideline notes a PSA density cut-off of 0.15 ng/mL/cc as predictive of clinically significant cancer.3

How to use our Prostate Volume Calculator

What Is Prostate Volume?

The prostate is a small gland in men. It sits under the bladder and wraps around the urethra, the tube that carries urine out of the body. Prostate volume is the size of that gland, measured in milliliters (mL). One mL is the same as one cubic centimeter (cm³).

A healthy young adult prostate is roughly the size of a walnut. The gland often grows with age. When it gets too big, it can press on the urethra and make it hard to pee.

How Prostate Volume Is Measured

A doctor uses an imaging scan to measure three lengths of the gland:

  • Length (L): top to bottom, seen from the side view
  • Width (W): side to side, the widest part
  • Height (H): front to back, also called anteroposterior

These three numbers usually come from a transrectal ultrasound (TRUS), an MRI, or a CT scan. The scan gives the sizes in centimeters or millimeters. Then a formula turns those three lengths into a volume.

The Two Formulas

Ellipsoid formula: Volume = (π ÷ 6) × L × W × H. This treats the prostate like a smooth egg shape. It is the oldest and most common method.

Bullet formula: Volume = (5π ÷ 24) × L × W × H. This treats the gland like a bullet, round on one end and flatter on the other. The bullet answer is about 25% larger than the ellipsoid answer.

The evidence is split. A 2009 planimetry study found the bullet shape fit most glands better, with accuracy particularly improved below 55 mL.1 A 2025 comparison against actual prostatectomy specimens found the opposite: the bullet formula significantly overestimated volume, while the ellipsoid estimate tracked the true volume closely.2 Discuss with your urologist which estimate fits your situation.

An enlarged prostate is called benign prostatic hyperplasia, or BPH. "Benign" means it is not cancer. Size helps doctors pick a treatment. Bigger glands often respond better to certain drugs, and very large glands may need a different type of surgery.

What Is PSA Density?

PSA stands for prostate-specific antigen. It is a protein made by the prostate that shows up in a blood test. A big prostate makes more PSA even when nothing is wrong. So a high PSA alone can be confusing.

PSA density fixes that problem. It divides the PSA level by the prostate volume:

PSA Density = PSA (ng/mL) ÷ Prostate Volume (mL)

The answer is in ng/mL². A value of 0.15 ng/mL² or higher is the cutoff the European Association of Urology's guideline describes as predictive of clinically significant cancer.3 It suggests the PSA may be high because of cancer, not just because the gland is large. A value below 0.15 more often points to simple enlargement. Doctors use PSA density, along with age, exam findings, and MRI results, to decide if a biopsy is needed.

Why This Number Matters

Prostate volume is used to track growth over time, plan surgery, calculate radiation doses, and judge how well a treatment is working. It also helps explain urinary symptoms like a weak stream, waking up at night to pee, or trouble starting. Frequently asked questions

Where do I find my prostate measurements?

Look at your ultrasound, MRI, or CT report. The three numbers are usually listed together, like 4.0 x 4.5 x 3.5 cm. They may be labeled length, width, and height, or sagittal, transverse, and anteroposterior (AP).

Does the order of L, W, and H matter?

No. Both formulas multiply the three numbers together, so the order does not change the answer. Still, enter them in the right boxes so the scaled diagram looks correct.

Which result should I use, ellipsoid or bullet?

The ellipsoid volume drives the size class and PSA density here, because those reference ranges were built on ellipsoid measurements; the bullet value is shown for comparison. A 2009 study favoured the bullet shape, especially under 55 mL,1 while a 2025 comparison against surgical specimens found the ellipsoid tracked true volume more closely.2 Many radiology reports use the ellipsoid number, so check which one your report used before comparing.

Why is the bullet volume always bigger?

The bullet formula uses 5π/24 (about 0.654) and the ellipsoid uses π/6 (about 0.524). That makes the bullet answer 25% larger every time, no matter what numbers you enter.

Is 1 mL the same as 1 cm³?

Yes. They are exactly equal. So a prostate volume of 40 mL is also 40 cm³. Some reports write cc, which means the same thing too.

Why are there two PSA density numbers?

One uses the ellipsoid volume and one uses the bullet volume. Because the bullet volume is bigger, its density is lower. Most doctors use the volume method their own report lists, so check both.

What does a PSA density under 0.15 mean?

It suggests your PSA is likely high because the gland is big, not because of cancer. A value of 0.15 ng/mL² or higher raises more concern. This is only one clue. Your doctor looks at age, exam, and MRI too.

What is a normal prostate volume by age?

A young adult prostate is roughly walnut-sized, and the gland often grows slowly from middle age onward. Growth alone is normal and does not always cause problems.

My volume is normal but I still have trouble peeing. Why?

Size is not everything. A small gland can still block the urethra if it grows inward. Bladder muscle problems, nerve issues, and medicines can also cause symptoms. Talk to your doctor.

What does the diagram show?

It draws two scaled views of the gland from your entries. The left one is the side (sagittal) view with L and H. The right one is the top-down (transverse) view with W and H. The shapes change as you change your numbers.

Why do I get an error message?

Each axis must be between 0.1 cm and 25 cm (1 mm to 250 mm, or about 0.04 to 9.8 inches). PSA must be between 0 and 10,000 ng/mL. Check for typos, blank boxes, or a unit set to the wrong choice.

Does this work for transition zone volume?

Yes. Enter the three transition zone measurements instead of the whole gland. The formulas work the same way. Just remember the size classification table is meant for total prostate volume.

How often should prostate volume be checked?

There is no set rule. Doctors often recheck when symptoms change, before surgery, or during long-term BPH treatment. Your doctor decides the timing.
