Introduction
The LV Mass Calculator works out how much your left ventricle weighs. The left ventricle is the main pumping chamber of the heart. When it works too hard, its walls get thicker and heavier. Doctors call this left ventricular hypertrophy (LVH). LVH raises the risk of heart failure, heart attack, and stroke.
This tool gives you two ways to find LV mass:
- MESA (cardiac MRI): You enter the LV mass that the MRI scan measured. The tool compares it to the mass expected for your sex, height, and weight.
- Devereux / ASE (echo): You enter three wall and chamber measurements from an echocardiogram. The tool works out LV mass from them.
You get four numbers back: total LV mass, LV mass index by body surface area, LV mass index by height2.7, and relative wall thickness with the heart's shape pattern. Each result is matched to the normal ranges for men and women from the ASE/EACVI 2015 guidelines. Charts, a reference table, and a step-by-step solution show you how every number was found.

How to use our LV Mass Calculator
Enter your patient's sex, height, weight, and heart measurements from a cardiac MRI or an echocardiogram. The calculator gives you left ventricular (LV) mass, LV mass index by body surface area and by height2.7, and tells you if there is left ventricular hypertrophy (LVH).
Calculation Method: Pick MESA (CMR-Derived) if you have an LV mass value from a cardiac MRI report. Pick Devereux / ASE (Echo) if you have wall and chamber measurements from an echo.
Biological Sex: Choose male or female. Sex changes the formula coefficient and the normal LV mass ranges used to grade the result.
Age (years): Type the patient's age, from 18 to 100. This field is optional and is used only to place the result next to adult reference data.
Height: Type the patient's standing height and pick the unit: cm, m, inches, or feet plus inches. Height is used for body surface area and for the height2.7 index.
Weight: Type the patient's body weight and pick kg, pounds, or stone. Weight is used for body surface area and for the MESA predicted LV mass.
Measured LV Mass (CMR): For the MESA method only, type the end-diastolic LV mass from the MRI report and pick g, kg, oz, or lb.
IVSd, septal thickness (diastole): For the echo method, type the septal wall thickness at end-diastole in mm, cm, or inches.
LVIDd, internal diameter (diastole): For the echo method, type the LV cavity diameter at end-diastole, measured just below the mitral leaflet tips.
LVPWd, posterior wall (diastole): For the echo method, type the posterior wall thickness at end-diastole, from the same frame as LVIDd.

What Is Left Ventricular Mass?
The left ventricle is the main pumping chamber of your heart. Left ventricular mass (LV mass) is the weight of the muscle in that chamber, measured in grams. Doctors find it with a heart ultrasound (echocardiogram) or a cardiac MRI (CMR).
Why LV Mass Matters
When the heart has to work harder than normal, the muscle gets thicker. This is called left ventricular hypertrophy, or LVH. High blood pressure, a tight aortic valve, obesity, and some genetic heart diseases can all cause it. LVH is a warning sign. People with high LV mass have a greater chance of heart failure, heart attack, stroke, and irregular heart rhythms.
How LV Mass Is Worked Out
- Devereux (ASE) formula, echo: Uses three wall and cavity measurements taken at end-diastole (when the heart is full): septal thickness (IVSd), inside diameter (LVIDd), and back wall thickness (LVPWd).
- MESA method, cardiac MRI: Takes the LV mass already measured on the MRI and compares it to the mass expected for someone of the same sex, height, and weight. A result above 136% of predicted counts as LVH.
Why the Number Gets Indexed
Big people have bigger hearts, so a raw gram number alone is not fair to compare. LV mass is divided by body size to make an index:
- LVMi by BSA (g/m²): Divides by body surface area. Normal is about 115 g/m² or less for men and 95 g/m² or less for women.
- LVMi by height2.7 (g/m2.7): Divides by height only. This works better for people with obesity, because body surface area can hide LVH in heavier patients.
Relative Wall Thickness and Heart Shape
Relative wall thickness (RWT) compares wall thickness to cavity size. With a cut-off of 0.42, it sorts the heart into four shapes:
- Normal geometry: normal mass, normal RWT.
- Concentric remodeling: normal mass, thick walls, small cavity.
- Eccentric hypertrophy: high mass, stretched cavity.
- Concentric hypertrophy: high mass and thick walls, the pattern with the highest risk.
When LVH Is Found
Thick heart muscle is not always from blood pressure. Other causes include hypertrophic cardiomyopathy (HCM), cardiac amyloidosis, Fabry disease, and athlete's heart from heavy training. These need different treatment, so a cardiologist should look at the full picture (symptoms, ECG, blood tests, and imaging) before deciding what is going on.
