Introduction

The LV Mass Calculator works out how much your left ventricle weighs. The left ventricle is the main pumping chamber of the heart. When it works too hard, its walls get thicker and heavier. Doctors call this left ventricular hypertrophy (LVH). LVH raises the risk of heart failure, heart attack, and stroke.

This tool gives you two ways to find LV mass:

  • MESA (cardiac MRI): You enter the LV mass that the MRI scan measured. The tool compares it to the mass expected for your sex, height, and weight.
  • Devereux / ASE (echo): You enter three wall and chamber measurements from an echocardiogram. The tool works out LV mass from them.

You get four numbers back: total LV mass, LV mass index by body surface area, LV mass index by height2.7, and relative wall thickness with the heart's shape pattern. Each result is matched to the normal ranges for men and women from the ASE/EACVI 2015 guidelines. Charts, a reference table, and a step-by-step solution show you how every number was found.

How to use our LV Mass Calculator

Enter your patient's sex, height, weight, and heart measurements from a cardiac MRI or an echocardiogram. The calculator gives you left ventricular (LV) mass, LV mass index by body surface area and by height2.7, and tells you if there is left ventricular hypertrophy (LVH).

Calculation Method: Pick MESA (CMR-Derived) if you have an LV mass value from a cardiac MRI report. Pick Devereux / ASE (Echo) if you have wall and chamber measurements from an echo.

Biological Sex: Choose male or female. Sex changes the formula coefficient and the normal LV mass ranges used to grade the result.

Age (years): Type the patient's age, from 18 to 100. This field is optional and is used only to place the result next to adult reference data.

Height: Type the patient's standing height and pick the unit: cm, m, inches, or feet plus inches. Height is used for body surface area and for the height2.7 index.

Weight: Type the patient's body weight and pick kg, pounds, or stone. Weight is used for body surface area and for the MESA predicted LV mass.

Measured LV Mass (CMR): For the MESA method only, type the end-diastolic LV mass from the MRI report and pick g, kg, oz, or lb.

IVSd, septal thickness (diastole): For the echo method, type the septal wall thickness at end-diastole in mm, cm, or inches.

LVIDd, internal diameter (diastole): For the echo method, type the LV cavity diameter at end-diastole, measured just below the mitral leaflet tips.

What Is Left Ventricular Mass?

The left ventricle is the main pumping chamber of your heart. Left ventricular mass (LV mass) is the weight of the muscle in that chamber, measured in grams. Doctors find it with a heart ultrasound (echocardiogram) or a cardiac MRI (CMR).

Why LV Mass Matters

When the heart has to work harder than normal, the muscle gets thicker. This is called left ventricular hypertrophy, or LVH. High blood pressure, a tight aortic valve, obesity, and some genetic heart diseases can all cause it. LVH is a warning sign. People with high LV mass have a greater chance of heart failure, heart attack, stroke, and irregular heart rhythms.

How LV Mass Is Worked Out

  • Devereux (ASE) formula, echo: Uses three wall and cavity measurements taken at end-diastole (when the heart is full): septal thickness (IVSd), inside diameter (LVIDd), and back wall thickness (LVPWd).
  • MESA method, cardiac MRI: Takes the LV mass already measured on the MRI and compares it to the mass expected for someone of the same sex, height, and weight. A result above 136% of predicted counts as LVH.

Why the Number Gets Indexed

Big people have bigger hearts, so a raw gram number alone is not fair to compare. LV mass is divided by body size to make an index:

  • LVMi by BSA (g/m²): Divides by body surface area. Normal is about 115 g/m² or less for men and 95 g/m² or less for women.
  • LVMi by height2.7 (g/m2.7): Divides by height only. This works better for people with obesity, because body surface area can hide LVH in heavier patients.

Relative Wall Thickness and Heart Shape

Relative wall thickness (RWT) compares wall thickness to cavity size. With a cut-off of 0.42, it sorts the heart into four shapes:

  • Normal geometry: normal mass, normal RWT.
  • Concentric remodeling: normal mass, thick walls, small cavity.
  • Eccentric hypertrophy: high mass, stretched cavity.
  • Concentric hypertrophy: high mass and thick walls, the pattern with the highest risk.

When LVH Is Found

Thick heart muscle is not always from blood pressure. Other causes include hypertrophic cardiomyopathy (HCM), cardiac amyloidosis, Fabry disease, and athlete's heart from heavy training. These need different treatment, so a cardiologist should look at the full picture (symptoms, ECG, blood tests, and imaging) before deciding what is going on.

Frequently asked questions

What is a normal left ventricular mass in grams?

Using echo measurements, the ASE/EACVI 2015 guidelines give these normal ranges for adults:

  • Men: about 88 to 224 g
  • Women: about 67 to 162 g

The raw gram number depends a lot on body size, so doctors usually look at the indexed value instead. Normal LV mass index is 115 g/m² or less for men and 95 g/m² or less for women.

How thick should heart walls be on an echocardiogram?

At end-diastole, normal wall thickness is:

  • Men: 6 to 10 mm
  • Women: 6 to 9 mm

Walls over 11 mm suggest thickening. Walls of 15 mm or more raise concern for hypertrophic cardiomyopathy or an infiltrative disease like amyloidosis, not just high blood pressure.

Can left ventricular hypertrophy be reversed?

Often, yes. If the cause is high blood pressure, lowering the pressure can shrink the muscle back over months to a few years. Weight loss, less salt, and treating sleep apnea also help. Surgery for a tight aortic valve can reverse it too.

LVH from hypertrophic cardiomyopathy is genetic and does not go away, but it can be managed.

Which blood pressure medicines reduce LV mass the most?

ACE inhibitors, ARBs (like losartan), and calcium channel blockers shrink LV mass the most. Beta blockers help less. Diuretics fall in the middle.

What matters most is getting blood pressure to target and keeping it there. Never change or stop heart medicine without asking your doctor.

Is LVH the same as an enlarged heart?

No. LVH means the heart muscle wall is thicker and heavier. An enlarged heart (cardiomegaly) usually means the heart chamber is stretched wider.

They can happen together. Thick walls with a small cavity is concentric hypertrophy. A stretched cavity with extra mass is eccentric hypertrophy.

How is athlete's heart different from LVH from disease?

Athlete's heart comes from heavy training. The walls thicken a bit, but the cavity also grows, the heart relaxes normally, and pumping stays strong. Wall thickness is usually under 13 mm.

Disease-related LVH often has a small or stiff cavity, poor relaxation, ECG changes, and symptoms. Athlete's heart usually shrinks after a few months of rest, which is one way doctors tell them apart.

What are the symptoms of left ventricular hypertrophy?

Many people have no symptoms at all and only find out from a scan. When symptoms do show up, they can include:

  • Shortness of breath, especially with activity or when lying flat
  • Chest pain or tightness
  • Fast or pounding heartbeat
  • Dizziness or fainting
  • Getting tired easily

Fainting during exercise needs urgent medical checking.

Can an ECG detect left ventricular hypertrophy?

An ECG can suggest it, but it misses many cases. ECG criteria such as Sokolow-Lyon or Cornell voltage catch only about 30 to 50 out of every 100 people who truly have LVH.

An echocardiogram or cardiac MRI is far better because it measures the wall thickness and mass directly.

Is cardiac MRI or echo better for measuring LV mass?

Cardiac MRI is the gold standard. It traces the real shape of the heart, so it is more accurate and repeatable, especially for odd-shaped or very thick hearts.

Echo is cheaper, faster, and widely available. But the Devereux formula assumes the ventricle is a smooth ellipse, so it can overestimate mass in big hearts and be thrown off by small measuring errors, since the numbers are cubed.

Why is LV mass indexed to height to the power of 2.7?

Body surface area includes weight, so in people with obesity the index can look normal even when the heart muscle is clearly too heavy. That hides real LVH.

Height2.7 uses height only, so extra body fat does not mask the problem. Cut-offs are 48 g/m2.7 for men and 44 g/m2.7 for women.

What does mild LVH mean and should I worry?

Mild LVH means the LV mass index is just above normal: 116 to 131 g/m² for men, or 96 to 108 g/m² for women. It is an early warning, not an emergency.

It still raises long-term risk of heart failure and stroke, so it is worth acting on. Controlling blood pressure, losing extra weight, cutting salt, and staying active can bring it back down.

Why does concentric hypertrophy carry the highest risk?

In concentric hypertrophy the walls are thick and the cavity is small. The heart becomes stiff and cannot fill well, so pressure builds up in the lungs. The thick muscle also needs more blood but has fewer vessels per gram, which can cause chest pain.

Of the four geometry patterns, this one links to the highest rates of heart failure and death.

Does losing weight lower LV mass?

Yes. Extra body weight makes the heart pump more blood, which adds muscle over time. LV mass drops after major weight loss from diet or bariatric surgery, often by 10 to 20 percent.

Treating sleep apnea, which is common with obesity, helps too.

What does 136% of predicted LV mass mean?

The MESA study built a formula that predicts LV mass from sex, height, and weight. Your measured MRI value is then compared to that prediction.

A result above 136% sits near the 95th percentile, so it counts as LVH. In MESA, people above that line had about 2.7 times the risk of heart attack, 5.4 times the risk of heart failure, and 7.5 times the risk of other cardiovascular death over 15 years.

Why are normal LV mass values different for men and women?

Men are usually taller and heavier and have more muscle overall, so their hearts weigh more even after adjusting for body size. Hormones also play a part.

Because of this, every threshold is sex-specific. A mass index of 100 g/m² is normal in a man but counts as mild LVH in a woman.

How long does it take for a thick heart wall to get thinner?

With good blood pressure control, measurable shrinking usually starts around 6 months and keeps going for 1 to 3 years. Most doctors repeat an echo after about a year to check progress.

How much it improves depends on how long the LVH was present, how well the pressure is controlled, and the cause.
